What I Do

I provide personalized, one-on-one physical therapy and wellness programs in the comfort of your home for adults with neurological conditions, including Parkinson’s, stroke, brain injury, dementia, multiple sclerosis, and vestibular disorders, as well as those experiencing weakness or increased fall risk.

Care is tailored to your goals and focused on improving mobility, balance, safety, and confidence in your everyday life. 

Services are currently private pay.

My Mission

My mission is to enhance mobility, strength, and overall quality of life for adults with neurological conditions through personalized, evidence-based physical therapy.

I focus on long-term solutions that support continued progress, independence, and resilience, extending beyond traditional therapy models and insurance limitations.

Skilled Physical Therapy (Mobile Outpatient-Self Pay)

One-on-one, in-home physical therapy focused on improving mobility, balance, strength, and overall function.

Initial Physical Therapy  Evaluation (60-90 minutes)

A comprehensive in-home physical therapy evaluation to assess movement, strength, balance, gait, and functional limitations.
A clinical assessment is completed to determine need for skilled physical therapy services and establish an individualized plan of care.

Physical Therapy Follow-Up Sessions (45-60 minutes)

One-on-one skilled physical therapy sessions delivered in the home, focused on progression of your individualized plan of care.Treatment is tailored to your specific clinical needs and may include therapeutic exercise, neuromuscular re-education, gait training, and functional mobility training as appropriate.Sessions may also include condition-specific interventions such as post-operative rehabilitation, vestibular rehabilitation, and caregiver education when clinically indicated.

Wellness / Guided Exercise Program
(Self-Pay) (Non-Skilled Services):

Personalized exercise and wellness sessions focused on building strength, balance, endurance, and confidence for everyday movement and fitness.

These sessions are designed for individuals who want structured support for exercise and to independently maintain an active lifestyle, including those with a history of physical therapy.

Sessions may include: 

  • ​Progressive strengthening

  • Walking and endurance training

  • Balance and coordination activities

The emphasis is on exercise performance, consistency, and long-term physical activity, rather than medical treatment or rehabilitation.

For Individuals with Medicare
Medicare regulations do not allow covered physical therapy services to be provided on a private pay basis. At this time, we are unable to offer private-pay skilled physical therapy services to Medicare beneficiaries. 

The Wellness and Exercise programs are non-medical service and are not covered by Medicare. Medicare beneficiaries may
participate in the Wellness programs on a self-pay basis.

About Me

Kristi Moore, PT, DPT, CSRS

I’m a physical therapist dedicated to helping adults with neurological conditions improve strength, balance, mobility, and endurance so they can move with more confidence and stay independent.

 

I earned my Doctorate in Physical Therapy from the MGH Institute of Health Professions after completing my Master’s in Physical Therapy at UCSF. I’ve worked in a variety of healthcare settings, shaping my approach to delivering personalized, effective care.

As a Certified Stroke Rehabilitation Specialist (CSRS), I bring specialized knowledge and experience into my work. I also work with individuals with vestibular conditions and dementia, using evidence-based assessments and treatments to create meaningful, lasting progress.

Outside of work, I enjoy running, weight training, traveling, hiking, and spending time with family and friends.
